INSURANCE Medical insurance is not accepted in our office and we cannot assist you with claim resolution. You are responsible for payment of all lab costs, tests, and prescription medications to be filled as well as any other health expenses you might incur outside our office.
LAB TESTS The lab will send all ordered blood work, tests, etc to your insurance company for insurance reimbursement. Lab tests that involve stool, urine or saliva samples are done by you in your home. You will be provided with all lab kits and step-by-step instructions for at home testing at the time of your appointment. Once all of the final lab results are received, your medical provider will review them at your follow-up visit, unless otherwise instructed.
FEES Your fees for service are dependent upon the level of service and individual components. All payments are due at the time of service. We accept Care Credit financing, cash, personal checks and all major credit cards. If you choose to use Care Credit Financing, you must purchase a minimum of $600. There is a $30 processing charge on all returned checks. You will be given a copy of your labs at each visit to keep for your records. Should you need additional copies of your medical records, a $25 fee will be charged for copies and postage.
CANCELLATIONS Aesthetic Appointment: There will be $20/hour charge for appointments not cancelled within 24 hours. Medical Appointment: There will be $50 charge for appointments not cancelled within 24 hours.
LATE ARRIVAL APPOINTMENTS If you arrive more than 15 minutes late to your appointment, you may be asked to reschedule your appointment.
REFUNDS Your satisfaction is our top priority. Aesthetic packages purchased are non-refundable and we do not issue cash refunds. If you are not satisfied with one of your products, please bring it back along with your receipt within one week of the purchase and we will gladly exchange the full value of the returned product for another product(s) that are more suited to your desired result. If you do not wish to have a product exchange, we would be happy to give you a credit towards one of our services or it may be placed as a credit on your account.
PHONE CALLS AND MESSAGES Phone messages left on our nurses line will be addressed within 48 hours (during business hours). If you call after hours, the office staff will return your call on the next business day. If you have a medical emergency, call 911 or go directly to the nearest ER.
PRESCRIPTION REFILLS For prescription refills, you need to contact your pharmacy and have them fax over the medication refill request. It may take up to 72 business hours to process a prescription refill. Please plan ahead to avoid any interruptions in your medications.
PRIVACY Solutions Medical Center is not a covered entity under HIPAA, but we take the privacy of your information very seriously. We make it a top priority to safeguard all of your information and to limit uses and disclosures of your information to only those purposes that you authorize as required by law. If you are over 18 years of age, you will need to sign a medical information release.
GIFT CERTIFICATES Gift Certificates are available for any service, package or dollar amount. Gift Certificates are not cash refundable.
