Interested in becoming a patient?

• Download and complete our new patient packet, or call our office at 901-853-6428, and we’ll be happy to email it to you. If you have any questions, feel free to call us at the same number, and one of our patient care coordinators will be glad to assist you.

• Once your packet has been returned and approved, then a patient care coordinator will call to schedule your extensive New Patient Consultation appointment. We require a non-refundable deposit of $200 to secure the appointment, which will be applied toward a medical plan.

• Following your consultation and blood drawn at our facility, you will meet with a patient care coordinator to discuss the medical plan recommended by your provider as the best fit for you. They will also help schedule your lab review appointment, any additional follow-up appointments, and address any questions you may have.
